RELEASE AGREEMENT by and between XXXX. and it's officer's, jointly and severally, both singular and plural, hereafter collectively called the RELEASEE and ___________________________________________, jointly and severally, hereafter called the RELEASOR, also known by any one of the following aliases, stage names, nicknames, maiden name, or professional name:

Be it known, that for good and valuable consideration and in consideration of the sum of $___________, the receipt of which is hereby acknowledged, the RELEASOR does hereby unconditionally and irrevocably release, discharge, acquit and forgive RELEASEE from all actions, causes of action, suits, debts, liabilities, contracts, obligations, controversies, judgements, executions, claims and demands both in law and in equity which the RELEASOR has or may have against the RELEASEE, whether asserted or unasserted, by reason of any matter or act whatsoever from the beginning of time to date hereof. This release shall, without limiting the generality of the foregoing further constitute full release, discharge and settlement of the following:


The use of any photographs, either by still photographic camera methods, or by the use of moving photographic methods, more commonly known as video taping, by use of one or more photographic camera, and/or, one or more video recorders, more commonly known as a camcorder. I understand that there may, or may not be, the use of more that one camera, or type of camera, or the use of one or more camcorders, or type of camcorder.

I understand that I may be photographed or videographed, at different times and places, in public or private, either clothed, semi-clothed, semi-nude, or nude, and at such times, I may or may not be engaged in a sexual act. I hereby state that if I choose to engage in an sexual act while performing as a paid model, it will be for my personal enjoyment and I have no objections to these sexual acts being photographed or videographed. I agree that if I chose to engage in a sexual act while performing as a paid model, it will only be a sexual act with a person, or persons, of my choosing and I agree to only perform such acts that I enjoy. I understand that I am not being paid to perform any sexual act upon any person or persons, I fully understand that the purposes of these photographs or videographs are of a sexually oriented nature, and may well be sold to others who also consider the said photographs or videographs, to be of a sexually oriented nature, and/or, appeal to a prurient interest. I consider myself to be a paid model, and having photographs or videographs taken of me, in either a clothed or nude state, or while engaging in any sexual act, does not trouble or distress me, and I have no interest whatsoever in how my likeness, either clothed or nude, is shown, displayed, sold or given to others. I also understand that my voice may be recorded by the camcorder, or by other means, and I have no interest whatsoever in how the recording of my voice may be used, broadcast, sold or given to others, or included on a video recording that may be shown displayed, sold or given to others.

I hereby give XXXX, their heirs, successors and assigns the absolute right and permission to publish, copyright, distribute, sell or give away, the use of my likeness, and/or voice, in either photographs or videographs or all other means of reproducing or duplicating the aforementioned photographs or videographs in whole or part, composite or distorted in character or form, in conjunction with my own or a fictitious name with reproduction thereof in color or otherwise, made through any media for art, adverting, media, or any other lawful purpose whatsoever.

I hereby waive any right that I may have to inspect and/or approve the finished product, type of product, or subject matter that may be used in connection therewith, or to the use to which it may be applied. I agree that in the event that I decline to continue a modeling session or decline to do future modeling sessions, any and all photographs or videographs made prior to my discontinuance, and the act of my discontinuance, shall be binding to the above release. I fully understand that I am not required to model in any manner whatsoever, or engage in any acts, of any type, that I find objectionable. I state that I will refuse to model in any manner I find objectionable, and refuse to perform any acts. of any type that I find objectionable. My continuance to model, and./or perform any acts of any type, indicate my total acceptance of the modeling assignment, my total acceptance of this release agreement, and my total, and complete lack of any objection, to any acts, or type of acts, that I might tend to engage in while performing this modeling assignment.

This release shall be binding upon and insure to the benefit of the parties, their successors, assigns, executors, administrators, personal representatives and heirs. This release constitutes the full agreement and cannot be altered or modified unless in writing.

I here by represent that I am over the age of 18 years and I have carefully read this release prior to its execution. I have not been coerced to sign this release. I have not asked to have this release explained to me by any other person, as I fully understand it myself. I have been told by the RELEASEE that I have the right to have an attorney inspect this release, and the RELEASEE told me that it may be in my best interest to have an attorney review this release, prior to my signing this release.
